Our Nursing and Care Coordination team works with you to understand your needs and create a Person-Centered Services Care plan that's customized just for you.
Person-Centered Service Planning
Person-Centered Service Planning keeps our focus on the client, with them leading the direction of our home care services. We work with our clients to customize a specific plan for them that helps us ensure that their goals and needs for Personal Care are met consistently.

Ways to Pay

Private Pay
Private pay is an option for families who prefer to pay directly for in-home care services rather than using Medicaid or long-term care insurance. This allows for greater flexibility in scheduling and care planning.

Our private pay rates start at $30 per hour and may vary depending on the level of care and support needed.
We aim to maintain competitive, fair pricing while delivering reliable, compassionate care tailored to each client’s needs.

Long-term Care Insurance
Clients who have an active long-term care (LTC) insurance policy may use their benefits to help cover the cost of in-home care services.
​
Services are paid out-of-pocket upfront. We then provide a detailed, itemized invoice outlining the dates of service, hours provided, and type of care delivered so you can submit documentation directly to your long-term care insurance provider for reimbursement.
​
We recommend contacting your insurance company to confirm eligibility, coverage limits, and reimbursement requirements prior to starting services.
Our team is happy to provide any documentation needed to support your claim process.

Medicaid
(MyCare Ohio Waiver)
We proudly accept MyCare Ohio Medicaid waiver plans, including Molina, Anthem Blue Cross Blue Shield, CareSource, Passport, and Buckeye Health Plan.

If you are enrolled in one of these managed care plans, we can coordinate directly with your insurance case manager or hospital discharge planner to obtain service authorization and begin care.

Approved hours are determined by your insurance provider and may vary based on assessed need.
If additional hours are required, our Registered Nurse can conduct a complimentary comprehensive assessment to help advocate for appropriate service levels on your behalf.
